BHRT and Skin Aging: How Hormones Affect Your Face and What to Do About It

There is a conversation I have regularly in my consultation room that almost never happens anywhere else: the conversation about hormones.

A patient sits down  typically in her mid-40s to mid-50s, sometimes younger  and describes changes in her face that she can’t fully explain. Her skin feels different. Thinner, somehow. Less resilient. She’s losing volume in places she didn’t expect. Her jawline is softening. The texture of her skin has changed in a way that skincare products aren’t addressing. She’s had filler, and it doesn’t seem to last as long as it used to. And she’s not sleeping the way she used to.

She’s describing hormonal aging. And in most aesthetic consultations, nobody connects those dots for her.

It supports sebaceous gland function in a way that keeps skin naturally moisturized and supple without being oily. Progesterone is the primary hormone responsible for regulating sleep quality in women. It has a direct calming, sedative effect on the nervous system  it binds to GABA receptors in the brain, the same receptors targeted by sleep medications, producing a natural relaxation and sleep-promoting effect. When progesterone declines in perimenopause and menopause, women frequently experience disrupted sleep: difficulty falling asleep, waking in the night, early morning waking, and a loss of the deep, restorative sleep stages that are essential for cellular repair.

And here is why this matters profoundly for skin aging: sleep is when the skin repairs itself. During deep sleep, growth hormone is released, cellular regeneration accelerates, cortisol drops, and the inflammatory processes that drive skin aging are suppressed. Chronic sleep disruption  which is what progesterone decline produces  elevates cortisol, increases systemic inflammation, accelerates free radical production, and impairs the skin’s ability to repair the daily damage it accumulates.
