Sculptra vs Radiesse: Which Biostimulator Is Right for You?

If you've been researching non-surgical aesthetic treatments, you've likely come across the names Sculptra and Radiesse. Both are biostimulators  a category of treatments that, unlike traditional fillers, don't just add immediate volume but stimulate the body to produce its own collagen.

But while they share that philosophy, they are very different products. And choosing the right one can make a significant difference in your results.

What Is a Biostimulator — And How Is It Different from a Filler?

Hyaluronic acid fillers work like scaffolding: they fill space, add volume, and hydrate. Results are immediate, visible from day one, and reversible with hyaluronidase.

Biostimulators work differently: they act as a biological signal that activates fibroblasts  the cells responsible for producing collagen and elastin. Results are not immediate, but they are longer-lasting and improve the structural quality of the skin from within.

Think of fillers as paint and biostimulators as structural renovation. Both have their place and I frequently combine them in the same treatment plan.

Sculptra: The Long-Term Biostimulator

Sculptra (poly-L-lactic acid) is the biostimulator with the longest clinical history. It was originally FDA-approved for the treatment of facial lipoatrophy in HIV patients, and later for aesthetic use in facial volume loss.

How it works: Poly-L-lactic acid microparticles are injected into the deep dermis and subcutaneous tissue. The body recognizes them as a foreign body and generates a controlled inflammatory response that stimulates type I collagen production. The particles gradually degrade into water and carbon dioxide.

Results: Results are gradual  they develop over 3 to 6 months and can last up to 2 years or more. This is an advantage for those seeking a natural, progressive change, and a disadvantage for those who want immediate results.

Typical protocol: Generally 2 to 3 sessions spaced 4 to 6 weeks apart are required for optimal results.

Ideal for:

  • Generalized volume loss in the cheeks, temples, and malar area
  • Patients seeking natural, progressive results
  • Overall skin quality improvement (texture, firmness)
  • Long-term maintenance with fewer sessions
  • Younger patients who want to prevent aging before it becomes visible

Considerations: Requires post-treatment massage for several days to distribute the product evenly. Not reversible. Results take time to appear.

Radiesse: Immediate Volume and Lasting Stimulation

Radiesse (calcium hydroxyapatite) is unique because it offers two benefits in one: immediate volume and long-term collagen stimulation.

How it works: Radiesse contains calcium hydroxyapatite microspheres  a mineral similar to what makes up bone — suspended in a carrier gel. The gel provides immediate volume, while the microspheres stimulate collagen production. Over time, the gel is absorbed and the microspheres degrade, leaving new collagen in their place.

Results: Immediate and visible from day one, with progressive improvement over the following months. Duration averages 12 to 18 months.

Diluted Radiesse (Skinbooster): One of the most innovative applications of Radiesse is its diluted use — injected into the superficial dermis to improve skin texture, firmness, and luminosity on the face, neck, décolletage, and hands. This technique produces an overall improvement in skin quality without adding volume.

Ideal for:

  • Jawline definition and mandibular angle
  • Cheekbone projection
  • Correction of volume loss in the cheeks
  • Hand rejuvenation
  • Texture and firmness improvement with the diluted technique
  • Patients who want to see immediate results

Considerations: Not reversible. Not recommended for lips or very thin-skinned areas. Requires an injector experienced in the correct technique to avoid irregularities.

Which Do I Recommend and When?

In my practice, the choice between Sculptra and Radiesse depends on several factors:

I choose Sculptra when: the patient has generalized volume loss, is looking for a very natural and progressive result, has time to wait for results, or wants a long-term maintenance treatment with few sessions per year.

I choose Radiesse when: the patient wants to see immediate results, the goal is contour definition (jawline, cheekbones), or I want to improve overall skin texture with the diluted technique.

I combine them when: the treatment plan requires both global volume (Sculptra) and contour definition (Radiesse), or when I want to maximize collagen stimulation through different mechanisms.

I also frequently combine them with microneedling with exosomes to improve skin quality while restoring volume at the same time.

Are You a Candidate for a Biostimulator?

Biostimulators are an excellent option if:

  • You notice your skin has lost firmness and volume over time
  • You want natural results that don't look "done"
  • You're looking for a more lasting solution than traditional fillers
  • You're in your 30s, 40s, or 50s and want to invest in your skin quality long-term

They are not the best option if you're looking for immediate results for an upcoming event, or if you have certain autoimmune or clotting conditions that we will evaluate during your consultation.

How much do these treatments cost in NYC?

The cost varies depending on the number of vials and areas treated. Generally, Sculptra costs between $800 and $1,200 per vial, and Radiesse between $700 and $1,000 per syringe. During your consultation we provide an exact quote based on your personalized treatment plan.

Is there any downtime?

There may be mild swelling, bruising, or tenderness for 3–5 days. Most of my patients resume normal activities the next day. With Sculptra, post-treatment massage for 5 days is important.

At what age are biostimulators recommended?

There is no strict minimum age, but I generally recommend them starting at age 30–35 as a preventive treatment. They are especially effective in patients aged 40 to 60 with visible volume and firmness loss.

Are they safe for Latin and darker skin tones?

Yes. Both Sculptra and Radiesse have an excellent safety profile across all skin phototypes. Unlike some lasers or peels, there is no risk of post-inflammatory hyperpigmentation with these injectable treatments.
