I have a phone call I make regularly  usually around three to four weeks after a patient’s first exosome hair therapy session. It follows a pattern I recognize immediately: a voice that’s trying to stay calm, and a sentence that begins with “I think something might be wrong.”

The concern is always the same. They’re shedding more hair than usual. And since they came in specifically because they were worried about thinning, this feels like the worst possible outcome.

It isn’t. In my practice, I’ve come to think of that phone call as a reassuring sign. Here’s exactly what the biology says is happening in your scalp  and why shedding after exosome therapy is often evidence the treatment is working.

The Hair Cycle: Why Shedding and Growing Are the Same Process

To understand why exosome therapy can trigger temporary shedding, you first need to understand how hair actually grows. It doesn’t grow continuously  it grows in distinct phases, and those phases are sequential and involuntary.

Anagen is the active growth phase. Each strand in anagen is being actively produced by its follicle, and this phase lasts two to seven years depending on genetics, nutrition, and scalp health. Roughly 85–90% of your hair is in anagen at any given moment.

Catagen is a brief two-week transitional phase where the follicle signals the strand to stop growing and detaches from its blood supply.

Telogen is the resting phase. The strand sits inside the follicle for approximately three months before it’s released  shed  to make room for the new anagen strand already forming beneath it.

What this means: every hair that sheds naturally was in telogen. Shedding is not loss. It’s the final step before new growth begins.

What Exosomes Do to Dormant Follicles

Exosomes are extracellular vesicles  the signaling packages that cells use to communicate. When we inject exosome-rich solution directly into the scalp, those vesicles deliver growth factors, peptides, and RNA instructions that are absorbed by follicle cells.

The primary mechanism relevant to shedding is this: exosomes signal dormant or weakened follicles to shift out of telogen and re-enter anagen. That’s the therapeutic goal  we want follicles that have stalled in extended rest to begin actively growing again.

But here’s the detail that explains the concerned phone calls. A follicle cannot begin a new anagen cycle while a telogen strand is still sitting inside it. Before new growth can emerge, the old strand must be released.

Exosomes can accelerate and synchronize this transition across multiple follicles simultaneously. When treatment stimulates several follicles at once to begin a new cycle, they shed their telogen strands within the same window  producing a concentrated burst of shedding that wouldn’t have been as noticeable if the reset had happened gradually over months.

I call this a telogen reset: the follicle isn’t failing. It’s clearing space for what comes next.

The Timeline I See in My Practice

This is a realistic, phase-by-phase picture based on patients I treat at Dr. Cris Studio.

Weeks 1–4: The scalp is absorbing exosome signals. Some patients notice mild shedding that feels like “slightly more than usual.” Others notice nothing at all. Both are normal.

Weeks 3–8: This is the window where shedding is most noticeable, if it’s going to happen. The telogen reset is occurring  old strands releasing as follicles begin new anagen cycles. For patients with more advanced dormancy, this phase can feel alarming. I want you to know before it happens that it shouldn’t be.

Months 2–3: Shedding resolves. Early signs of new growth begin  usually fine, shorter strands that weren’t present before, often first visible at the hairline or temples.

Months 3–6: This is when most patients see the meaningful change in density. Hair shafts come in thicker because the follicle is producing them from a healthier growth phase. Volume returns noticeably.

Month 6+: Continued improvement, particularly for patients completing the 3-session protocol. Results tend to compound with each treatment.

One important note: not every patient experiences visible shedding. If you don’t, this doesn’t mean the treatment isn’t working. It may mean your follicles were in early telogen rather than deep dormancy, or that shedding happened gradually enough to go unnoticed. Absence of the shedding phase is not absence of response.

Who Is Most Likely to Notice It

Patients with more advanced or longer-standing thinning tend to experience a more pronounced shedding window. This makes biological sense: if follicles have been sitting in extended telogen, more of them may be primed to reset simultaneously.

Patients who come in proactively  early-stage thinning, preventive approach  often experience such mild shedding they don’t mention it at follow-up.

What I watch for: if a patient is still shedding heavily past the eight-week mark without any emerging signs of new growth, that warrants a closer look. We reassess the protocol and rule out contributing factors — iron deficiency, thyroid imbalance, chronic stress — that can counteract treatment response regardless of how well the exosomes did their job.

What to Do (and Not Do) During the Shedding Phase

Do photograph your hairline and part width before your first session. The shedding phase is disorienting without a baseline. Images from before treatment make it possible to objectively compare  and to see the improvement that follows once new growth comes in.

Don’t panic-buy new supplements or switch products mid-protocol. The follicles are doing exactly what we asked them to do. Introducing variables mid-cycle makes it harder to evaluate the treatment’s true response.

Don’t stop treatment after the first session because of the shedding window. This is the most common mistake I see  discontinuing precisely when the results phase is about to begin.
