Crepey Skin After Weight Loss: Why It Happens and How to Fix It

Crepey skin is one of the most common concerns I hear at Dr. Cris Studio — and one of the most undertreated. Patients describe it the same way every time: “my skin looks like tissue paper,” “it’s wrinkled even when I’m not moving,” “it started on my arms and now it’s on my neck, my chest, my inner thighs.”

Most of them have already tried every cream, oil, and firming lotion on the market. Some have helped a little. None have fixed it. And that’s not because they chose the wrong product  it’s because crepey skin is a structural problem, and topical products work at the surface level. You cannot moisturize your way out of collagen loss.

This post explains what crepey skin actually is at the biological level, what causes it, and what treatments  biostimulators, CO2 laser, and exosome therapy  can produce meaningful, lasting improvement.

What Crepey Skin Actually Is

This is why it looks the same whether you’re moving or still, and why surface-level treatments produce limited results.

What Causes Crepey Skin

Several factors contribute to collagen and elastin depletion  and most patients have more than one at play simultaneously.

Age

Collagen production peaks in the mid-20s and declines by approximately 1% per year thereafter. By the time most patients notice crepey skin  typically in their 40s and 50s  they have lost 20–30% of their peak collagen density. The skin becomes thinner, less resilient, and less able to maintain its smooth surface.

Hormonal Changes

Estrogen plays a direct role in collagen synthesis  it stimulates fibroblast activity and supports the skin’s structural protein production. The decline in estrogen during perimenopause and menopause accelerates collagen loss significantly: studies show that skin loses approximately 30% of its collagen in the first five years after menopause. Sun Damage

Decades of cumulative sun exposure  even without visible burning  produce significant dermal collagen loss, particularly on the arms, neck, and décolleté.

Weight Loss

Significant weight loss  whether from diet, exercise, or GLP-1 medications like Ozempic or Wegovy  removes the subcutaneous fat that provides structural support beneath the skin. When that support disappears faster than the skin can adapt, the result is crepey, loose skin that lacks the internal scaffolding to remain taut. The faster the weight loss, the more pronounced the crepiness. This is particularly visible on the arms, inner thighs, and abdomen.

Pregnancy

Pregnancy is one of the most common causes of crepey skin on the abdomen  and one of the most underrecognized. During pregnancy, the skin of the abdomen stretches rapidly and significantly to accommodate the growing uterus. This stretching can exceed what the collagen and elastin fibers can accommodate without damage: the elastin fibers fracture, reducing the skin’s ability to recoil after delivery. The result is the loose, crepey, sometimes wrinkled abdominal skin that many patients notice postpartum  even after the pregnancy weight has been lost.

Hormonal changes compound the problem. Estrogen  which directly supports collagen synthesis  drops significantly after delivery and during breastfeeding, reducing the skin’s capacity to rebuild the structural proteins that were damaged during pregnancy. For patients who have had multiple pregnancies, or who were pregnant later in life when collagen reserves were already declining, the cumulative effect on abdominal skin quality can be significant.

Dehydration and Lifestyle Factors

Why Topical Products Can’t Fix Crepey Skin

This is the question I get most often, and the answer is straightforward: topical products work at the epidermis  the outermost layer of the skin. Crepey skin is a dermal problem. The dermis sits beneath the epidermis, and most topical ingredients cannot penetrate deeply enough to reach it in concentrations sufficient to rebuild structural protein.

Retinol and certain peptides can stimulate some collagen production in the upper dermis with consistent long-term use  and they are worth using as part of a maintenance routine. But they cannot rebuild the collagen matrix that has been lost over decades of aging, sun damage, hormonal decline, weight loss, or pregnancy. For that, you need treatments that work at the dermal level directly.

What Actually Works: Treatments for Crepey and Flabby Skin
