Stem Cells, Exosomes & PRF: What's the Difference?

If you've been researching regenerative aesthetics in NYC  or even just scrolling past a clinic's Instagram  you've probably come across three terms: stem cells, exosomes, and PRF. They tend to appear together, often in ways that imply they're interchangeable. They're not.

Almost every week, a patient sits across from me and asks some version of the same question: "What's actually the difference, and which one do I need?" It's a fair question. And the honest answer is that most people explaining these treatments, even in clinical settings, blur the lines between them. They don't work the same way. The regulatory landscape around each one is very different. And the marketing often runs far ahead of the evidence.

As someone trained in maxillofacial surgery in Brazil  where PRF research has deep roots in the dental and craniofacial surgical world and currently practicing as a nurse injector in New York, I want to give you a clear-eyed guide to all three: what they are, what the evidence actually supports, and how I think about using them.

What All Three Have in Common

Stem cells, exosomes, and PRF all belong to the category of regenerative aesthetics: treatments that aim to activate or support your body's own tissue-renewal mechanisms, rather than simply replacing lost volume or relaxing muscle. The appeal is real. Instead of substituting what's been lost, you're prompting the body to rebuild it.

Where they differ is in how they trigger that rebuilding, how well-established the evidence is, and how they stand from a regulatory standpoint.

PRF (Platelet-Rich Fibrin): The Well-Established Foundation

PRF is the treatment I reach for first when a patient wants an autologous regenerative option  something derived from their own body, with a well-characterized evidence base.

Here's how it works. We draw a small amount of your blood and spin it in a centrifuge at a lower speed and without anticoagulants. The fibrin layer that rises to the top is rich in platelets, growth factors (PDGF, TGF-β, VEGF, IGF-1), and white blood cells. When injected or applied topically over microneedling channels, it creates a slow-release fibrin matrix that delivers those growth factors over days to weeks  triggering collagen synthesis, supporting tissue repair, and improving skin quality.

PRF evolved from PRP (platelet-rich plasma), an earlier protocol that used anticoagulants and delivered growth factors more rapidly. The fibrin matrix in PRF produces a more sustained, biologically active release. This difference matters: the slower, extended release of PRF appears to drive more robust collagen stimulation than PRP's immediate delivery, based on the research I followed through both my surgical training and my continuing education in aesthetics.

Because PRF is derived from your own blood, it sidesteps many of the regulatory questions that surround newer therapies. It's autologous, the protocol is standardized, and the safety profile is well understood. For patients who want regenerative treatment with a strong evidence base and no sourcing questions, PRF is typically where I start the conversation.

Exosomes: The Cellular Messengers

Exosomes are where regenerative aesthetics becomes genuinely exciting  and where I always slow down to make sure patient expectations are grounded in what the science actually shows, rather than the marketing.

Exosomes are nano-sized vesicles: membrane-enclosed packets secreted by cells — in aesthetic preparations, most often mesenchymal stem cells (MSCs)  carrying growth factors, signaling proteins, and microRNA. They're the messaging system between cells. They don't replace cells or generate tissue directly; they instruct surrounding cells to do so. When delivered into the dermis via microneedling channels, they can trigger collagen remodeling, reduce inflammatory signaling, and accelerate cellular turnover at a depth that topical products cannot reach.

An important point on regulation: As of 2026, exosomes have no FDA approval for aesthetic use in the United States. They exist in a regulatory gray area  neither clearly approved nor banned for cosmetic application when used alongside procedures like microneedling. I share this not to dissuade you, but because transparency is a core part of my job. Any provider who doesn't mention this is worth questioning.

What I can tell you from my own clinical practice: in my experience, the results with our microneedling combined with exosomes and PDRN protocol can be striking — improved skin texture, luminosity, and barrier function that outlasts what microneedling alone typically produces. The combination of exosome signaling with PDRN (salmon DNA, which provides nucleotide building blocks directly to skin cells for repair) is a dual-pathway protocol I find clinically compelling: one ingredient signals the repair, the other provides the substrate for it.

A note on my protocol: At Dr. Cris Studio, I do not combine exosomes with PRF or PRP in the same session. These are distinct regenerative pathways, and I prefer to keep them separate to maintain clarity on what's driving results, avoid unpredictable interactions between autologous and non-autologous preparations, and ensure each treatment is delivering its intended signal without interference. If you've seen clinics offering "exosomes + PRF" as a combined package, that's a clinical choice  but it's not mine, and I'll always explain why during your consultation.

Stem Cells: The Source, and Why Most "Stem Cell Treatments" Are Really Exosome Treatments

Here's the distinction that most marketing glosses over entirely: in most aesthetic "stem cell" treatments, you're not actually receiving living stem cells. You're receiving exosomes or growth factor concentrates derived from stem cells — typically mesenchymal stem cells sourced from umbilical cord tissue (Wharton's Jelly), bone marrow, or adipose fat, processed in a laboratory.

True stem cell treatments  where living, pluripotent cells are introduced to stimulate tissue regeneration  are largely investigational in aesthetic medicine. SVF (stromal vascular fraction) procedures, which use stem cells harvested from your own fat via liposuction, are performed by physicians under research or clinical protocols and are not standard medspa offerings.

This distinction matters for you as a patient. If a clinic markets "stem cell therapy" for skin or hair, ask specifically what product they're using and where it's sourced from. In most cases, the answer will reveal that what's being offered is an exosome preparation — a valid treatment, but not what "stem cells" implies to most people. Clarity about what you're actually receiving is reasonable to expect.

How I Use These at Dr. Cris Studio

These approaches aren't mutually exclusive across your treatment plan, but I keep each session focused on one regenerative pathway. Which One Is Right for You?

No blog post replaces a consultation, but here's the framework I use:

PRF if you want a time-tested, autologous regenerative option with a strong evidence base and no sourcing concerns. Well-suited for patients with sensitivities or those who prefer treatments derived entirely from their own biology.

Exosomes if you're interested in advanced cellular signaling  combined with microneedling  for skin or hair concerns, and you're comfortable with a protocol that shows promising clinical results alongside evolving evidence and an active regulatory conversation.

Be cautious about "stem cell" claims in a medspa context. Ask what product is actually being used. What's most likely being offered is an exosome preparation  which can be excellent, but deserves accurate labeling.

Six Questions I Get Asked About This Every Week

1. Are exosomes safe?
In clinical practice combined with microneedling, exosome preparations have shown a favorable safety profile in published studies and in my own patients to date. Because exosomes are not FDA-approved for aesthetic use, I disclose this clearly before every treatment and discuss it in the context of your individual history and goals. Adverse events reported in the literature are rare.

2. How is PRF different from PRP?
PRF uses a slower centrifuge speed and no anticoagulants, producing a fibrin matrix that releases growth factors gradually over days to weeks rather than as an immediate bolus. Most research suggests this sustained-release mechanism produces more robust collagen stimulation than PRP's faster delivery.

3. Can exosomes and PRF be combined?
At Dr. Cris Studio, no. I keep exosome and PRF/PRP sessions separate by design. Mixing autologous blood-derived preparations with non-autologous exosome products introduces variables I prefer to avoid  both for safety clarity and to ensure we can accurately attribute your results. Each modality is powerful on its own; combining them in a single session is not part of my protocol.

4. How many sessions do I need?
Individual response varies. For skin renewal, many patients see meaningful improvement after 2–3 sessions spaced 4–6 weeks apart. For hair concerns, 3–4 sessions over a similar window is typical. Results continue to develop over 3–6 months as collagen synthesis progresses  the full picture often isn't visible at the end of the final session.

5. Can I receive these treatments if I take blood thinners?
PRF requires a blood draw and is not appropriate for patients on anticoagulant therapy. Exosome treatments don't require blood draw and may be an option, depending on the specific anticoagulant and your full medication picture  this is always a case-by-case clinical conversation. Please share all medications at your consultation.

6. Are the results permanent?
Regenerative treatments stimulate your body's own repair mechanisms, but they don't stop the aging process. Results can typically be maintained with periodic treatment — annually or biannually depending on the modality and your individual response. Many patients build regenerative sessions into a consistent maintenance cadence alongside their injectables.
