What Exosome Hair Therapy Is

Exosomes are extracellular vesicles, tiny messengers that cells use to communicate with each other. The exosomes used in aesthetic medicine are derived from stem cell sources and contain a concentrated payload of growth factors, proteins, and signaling molecules that tell the scalp to regenerate. When applied to the scalp, typically via microneedling or direct scalp injection, they reach the follicle level and begin a process of cellular repair and stimulation that unfolds over the following weeks and months.

The result is not instant. Hair grows in cycles, and the visible improvement, thicker, denser hair with less shedding, usually becomes noticeable at the three to six month mark. Patients who want quick results are better served by other options. Patients who want lasting improvement in the health and density of their hair are often excellent candidates.

Who This Treatment Is For

I most commonly treat patients with androgenetic alopecia (pattern hair loss in both men and women), early to moderate thinning, postpartum shedding, and stress-related or diffuse hair loss. The treatment works best when there is still follicular activity to build on, which is why earlier intervention tends to produce better outcomes. If you are noticing a widening part or increased shedding and wondering whether you have waited too long, the answer is almost always: come in sooner rather than later.

What the Treatment Looks Like

The appointment takes about an hour. I begin with a scalp assessment, then apply topical numbing, then deliver the exosomes using a combination of microneedling and scalp injection, depending on your pattern of thinning. The scalp may be slightly pink or sensitive for twenty-four to forty-eight hours afterward. Most patients return to their normal routine the same day.

A single treatment can produce noticeable improvement, but a series of two to three sessions spaced four to six weeks apart typically delivers the most significant results. I also discuss maintenance, because hair loss is an ongoing process for most patients, and sustaining results usually involves periodic treatment over time.

Questions My Patients Ask

How is this different from PRP?

PRP uses your own blood, spun down to concentrate growth factors. Exosomes are derived from stem cell sources and contain a much higher concentration and broader range of signaling molecules. The mechanisms overlap but exosomes are generally considered more potent, particularly for patients who have not responded well to PRP. Why does hair shed before it grows?

A temporary increase in shedding in the four to eight weeks after treatment is actually a sign the follicles are cycling, clearing older hairs to make way for new growth. It is not cause for alarm. I explain this in detail in my post on why hair sheds before it grows after exosome therapy.

How many treatments do I need?

Most patients do a series of two to three treatments, then maintenance every six to twelve months. The exact protocol depends on the degree of thinning, how you respond, and whether you are on any other hair loss treatments. We discuss this at the consultation before you commit to anything.

Is it painful?

The scalp responds well to topical numbing. Most patients describe the sensation as pressure, occasionally a brief sting, and rarely what they would call pain. The discomfort level is typically lower than patients anticipate.

How soon will I see results?
